The Critical Role of Nutrition

Like any type of invasive procedure, surgical outcome tends to improve when the patient is in good physical health. Optimizing a patient's nutritional status is an important part of this process and is essential for proper wound healing even after non-invasive procedures.

Any surgically invasive or non-invasive cosmetic procedures including Botox®, filler injections and laser or chemical skin treatments damages tissues and initiates a cellular wound healing response. Wound healing sets off a complex chain of events that involves increased cellular activity and an intensified metabolic demand for nutrients. This activity requires a number of vitamins and other chemical cofactors to complete its job. Specifically, for a wound to heal successfully, patients need adequate amounts of several different types of nutrients, including carbohydrates, protein, fat, vitamins, and minerals.

Without these chemical building blocks, wound healing takes longer and may be compromised. Patients who don't meet their nutritional needs may be at risk for increased bruising and swelling, delayed wound healing, and other wound-related complications including an increased chance of infection. Proper wound healing also means, for example, that cells are better able to make and lay down the appropriate amounts of critical structural skin scaffolding molecules such as collagen and elastin. Enhanced cellular events such as these translate into a better, more optimal, cosmetic result after any procedure. In addition, several of the anesthetic agents that are used during surgery and the laser and chemicals used to treat the skin generate a significant number of free-radicals. These unstable oxygen molecules must be neutralized or eliminated by the body to prevent damage to healthy cells. Specific antioxidants, supplied in supplement form, can potentially enhance the intrinsic systems the body maintains for handling free radicals.

Welcome to Dr. Joe's Personal Trainer's Calorie Counter. Here we will first determine the your overall fitness shape as compared to others. This is called your body mass index (BMI) and is a good general indicator of your current fitness condition and your potential risk of disease from the consequences of being over your recommended weight.
Next we will determine the number of calories your body burns during the day. This is called your basal metabolic rate (BMR). This important number is the minimum number of calories your body needs to function correctly during a normal day. We will then use this number to formulate a diet and exercise plan to keep you under the recommended number of calories needed for safe rapid weight loss.

This is an exciting time for you. If you follow our simple recommendations you will start feeling great both inside and outside. At SliceLabs, LLC, makers of SliceCream, we not only invent all of our products but we eat them everyday as a living testimony to their effectiveness. Therefore, since I used the SliceCream diet principles to get ready for my honeymoon I know from a real life experience (please see my before and after pictures below). CALORIE ASSESSMENT

This test will evaluate your Body Mass Index (BMI) and then calculate your Basal Metabolic Rate (BMR). This information will be used to calculate the suggested number of daily calories to eat to lose weight. Feel free to read below for more information or simply
click on the Golden Manikin to launch the test. After filling in your information in the white box on the left, the yellow box on the right will show you the estimated number of calories you normally burn at rest and the number of suggested calories to consume in order to lose weight safely and effectively. This last number should be kept in mind when adjusting your weekly meal plan to the foods you like to eat. The estimated number of calories calculated to lose weight should also be kept in mind when determining the type of exercises you enjoy and number of calories they burn.
Its easy. To summarize, the number of calories you eat during the day minus the number of calories you burn with exercise in a day should not go over the total number of daily calories calculated to lose weight. This number, the total number of daily calories, for most women and men should be between 1500 and 1800 calories per day. If you end up eating a little bit too much during the day make sure they are "healthy foods" and then exercise a little more during the same day to get down to the estimated number of calories to lose weight. It may take a few days to get used to this type of calorie counting but you will soon begin to understand how your body responds and base your daily diet and exercise regime based on its response. Importantly, you should never go under 1500 calories per day. Doing so could deprive you of the important nutrients you need to ready your body for a cosmetic procedure and then hinder recovery from the procedure. Studies have shown that the body does most of its remodeling and rebuilding during rest so try to get enough sleep and also remember to drink plenty of low calorie fluids. Lastly, enjoy your free day! Eat what you like using common sense portions and lay off all exercise so that your body has time to rest and refuel.


For most individuals, if you follow these basic guidelines you will have an approximate daily calorie deficit of 500 calories. Since it takes 3500 calories to "burn off" 1 pound of body fat, you will lose at least 1 pound per week. However, since there are other factors that go into how the body controls metabolism, you could also end up losing up to 2 pounds per week if you follow the general guidelines. Each individual's metabolism level responds differently which is why it is useful to keep a very basic log of your progress. To do this, keep a pen and sheet of paper in the room next to your weight scale and keep track of your weekly progress. The body is a wonderful machine and it is better to watch it adapt to your program over weekly rather than daily intervals. If measured daily, some days you may not see a change in your weight while other days you may see more of a change. Because of this and to help avoid the non-motivating aspects of disappointment, it is NOT advisable to weigh yourself everyday but to do so every week. Again, if you stick to the general nutrition and general plan you will turn your body into a fat burning furnace and YOU WILL SEE RESULTS. For those patients undergoing liposuction, your physician can only remove a certain amount of fat safely. Keep reminding yourself that the reason you are following the SliceCream diet principles is so that the effects of your liposuction will be even more dramatic than liposuction alone. With the combination of your cosmetic procedure, good nutrition and exercise, It won't be long until your body is dramatically changed and your appearance and self confidence is boosted to its highest levels!

DEFINITIONS

Body Mass Index
Established by the federal government as a standard to determine obesity, Body Mass Index (BMI) is calculated by dividing your weight in kilograms by your height in meters squared. A Body Mass Index between 18.5 and 25 is ideal. People with a BMI between 25 and 29.9 are overweight and a BMI greater than 30 indicates obesity. Being overweight increases the chances of serious health risks like heart disease, diabetes, and high blood pressure. However, Body Mass Index is not a foolproof measurement. Muscle tissue weighs more than fat tissue and the BMI calculation does not recognize the difference between muscle and fat. However, even with this imperfection, the BMI calculation can be a useful first indicator of general fitness.

Basal Metabolic Rate
You expend energy no matter what you're doing, even when sleeping. Your Basal Metabolic Rate is the number of calories you would burn if you stayed in bed all day. If you are older you have probably noticed that every year it becomes harder to eat whatever you want and still stay slim. This occurs because your Basal Metabolic Rate decreases as you age. Importantly, depriving yourself completely of food in hopes of losing weight fast also decreases your BMR. However, a regular routine of cardiovascular exercise can increase your BMR and improve your health and fitness when your body's natural ability to burn energy at rest gradually slows down.

    Liposuction

    Liposuction is one of the most popular cosmetic surgical procedures and can help to restore a more pleasing shape to specific areas of your body by safely removing unwanted fat. The areas that can be improved through liposuction include the abdomen, thighs, hips, buttocks, knees, upper arms, chin, cheeks and neck. Liposuction is a great choice for average weight people with firm, elastic skin who have pockets of excess fat in specific areas that just want to have a slimmer, shapelier look. Since Liposuction can only safely remove a limited amount of fat, the liposuction procedure combined with a regimented diet and exercise program both before and after your procedure can greatly enhance your results in essence radically changing your body image. If followed closely, patients undergoing liposuction in combination with the safe and effective SliceCream diet plan can experience tighter, more toned, hips, buttocks and thighs and a "wash-board" abdomen with enhanced appearance of the abdominal muscles.     The Procedure

    Consuming the proper nutrition is one of the most important components of your preparation before liposuction and during recovery and maintenance after your liposuction procedure. The nutrients you put into your body during each phase of your procedure will have a profound impact on the results you achieve.     Exercise is also critical components of your preparation before liposuction and maintenance after liposuction. Liposuction will remove some fat away but for most individuals liposuction alone will not give you a super flat toned abdomen. Following an exercise plan prior to having your liposuction procedure will allow your physician to remove. Depending upon the location of the procedure, the more fat removed the better the definition of say the abdominal muscles. In the same way, exercise prior to liposuction will also give your thighs and buttocks a more defined "cut" look.     Dr. Leonard Miller developed "The Natural Facelift" following his 25 years experience in facial rejuvenation.     The Procedure - The operation is safer, less extensive and better tolerated than other traditional facelift procedures.     Eyelid Procedures

    Eyelid surgery can lift drooping upper lids and reduce the puffy bags that form below your eyes - features that may be making you look older and more tired than you feel. Some patients benefit from choosing to have other reshaping facial surgery performed at the same time as their eyelid surgery, including a forehead lift, face lift and/or improvements to the nose & chin.

    The Procedure - The procedure takes approximately two hours for both the upper and lower lids. An incision is made along the natural lines of the creases of your upper lids and just below the lashes in the lower ones. The skin is separated from the subcutaneous layers, the excess fat is removed, sagging skin is then trimmed. Very fine sutures are then used to close the incisions.
    For those patients only requiring pockets of fat removed beneath the lower eyelids, a transconjuctival blepharoplasty can be performed where an incision is made inside your lower eyelid so that no scar is visible on the face. Stitches are removed approximately 5 days after surgery. Once they're removed, any swelling and discoloration around your eyes will gradually subside.
    Eyelid plastic surgery can enhance your appearance and your self-confidence, but it will not give you ideal or perfect eyelids. Eyelid surgery cannot remove crow's feet or other deep wrinkles, eliminate dark circles beneath the eyes, or lift sagging eyebrows.     BREAST AUGMENTATION
    Breast Augmentation is a surgical procedure to enhance the size and shape of a woman's breast by inserting implants to increase a woman's bustline by one or more bra cup sizes. The Procedure - The method of inserting and positioning your breast implants will depend on your anatomy, preferences and Dr. Miller's recommendation. Working through an incision, Dr. Miller lifts the breast tissue and skin to create a pocket, either directly behind the breast tissue or underneath your chest wall muscle. The operation usually takes about 2-3 hours and can be performed with a local anaesthetic and monitored intravenous sedation at our fully-accredited surgical suite at One Brookline Place or a general anaesthetic at the hospital if your prefer. Recovery (What To Expect) - Most patients can return to work within a few days. Heavy lifting and strenuous workout routines should be avoided for a period of 1 month.     Breast Lift is the surgical raising and reshaping of the breasts to improve their appearance which may have been adversely affected by pregnancy, nursing and simple gravity. As the skin loses its elasticity, the breasts often begin to sag and a breast lift can help counter these effects.

    The Procedure - Breast lifts are usually performed under a local anesthetic with intravenous sedation and take approximately 3 hours for both breasts. There are many techniques available, but Dr. Miller commonly uses a crescent-shaped incision that follows the natural contour of the breast. The incision defines the area from which breast skin will be removed and determines the new location of the nipple. Dr. Miller will remove the excess skin and moves the nipple and areola up to the higher position. The skin around the areola is then trimmed and drawn together to reshape the breast. Other incision patterns may be more suitable for your treatment and Dr. Miller will discuss your alternatives. Often times patients will have an implant inserted along with their breast lift to increase their size, which is placed in a pocket directly under the breast tissue, or deeper under the muscle of the chest wall.

    Recovery (What To Expect) - Most patients are up and about in a day or two, but do not return to work for a week or more. Patients will need to wear a special support bra around the clock for three to four weeks

Eating right is critical so here are a few tips to help you make sure that you eat right.

    Tip #1 - Always Take a Healthy Lunch - First of all, always makes sure that you take a healthy lunch with you to work. Sure, eating out can be fun, but usually when you eat out the foods available are not so healthy for you. The last thing you want to do is eat out on a regular basis and sabotage your fitness program. So, take a lunch with you to work that is very healthy to compliment the program you are on.
    Tip #2 - Get Rid of Junk Foods - If you are working to lose weight and you are on a fitness program, getting rid of the junk food in your home is an important decision. Instead of just keeping it in a cabinet, totally get rid of it. When you have them around you are more likely to go and eat them, so just don’t purchase them at all.
    Tip #3 - Eat Smaller Portions - Eating smaller portions is very important as well when you are trying to lose weight and keep your fitness program on track. Going on a strict diet is not really the answer for most people but eating smaller amounts of all your foods is a great idea that will help you stay on track and lose weight.

    Breast cancer, Nutrition and lifestyle changes
    Breast cancer is the most common cancer in US women and like prostate cancer is hormone dependent. Although breast cancer is estrogen dependent, most breast cancer occurs in postmenopausal women in whom estrogen levels are low; however, elevated serum estradiol (a form of estrogen). has been shown to be a risk factor for breast cancer in postmenopausal women. The influence of lifestyle factors in breast cancer is supported by cross-cultural variation and migration studies. For example, breast cancer incidence and mortality for Asian immigrants in the United States is intermediate between US white women and Asian women who live in Japan or China, and these differences have been attributed in part to adoption of Western diets among Asian immigrants. Fat intake has been suggested as a contributor to breast cancer, with reports of a positive correlation between per capita fat consumption and both incidence and mortality from breast cancer. In one large retrospective study of 2,599 breast cancer patients and 2,588 controls, it was found that increased consumption of calories, saturated fat, sugar, and alcohol all significantly correlated with increased risk for breast cancer. The low breast cancer incidence in East Asian women is associated not only with dietary fat, but also with diets high in omega-3 fatty acids (i.e., fish oil) and fiber. In addition, Asian women may be more physically active. One meta study showed that 32 of the 44 studies to date have observed a reduction in breast cancer risk in women who were most physically active, and of the 32 studies that observed a decrease, the average risk reduction was 30–40%. Additionally, a dose-response relation between increasing activity and decreased risk was noted in 20 of 23 studies.

    Physical activity may modulate the production, metabolism, and
    excretion of sex hormones, as sedentary, postmenopausal women have higher serum estradiol and estrone and lower SHBG concentrations. Additionally, increased body fat, present in sedentary women, is a location of increased aromatizaton of steroid hormones. Sedentary women also have high insulin, which may be a promoter of breast tumor development.

    One study reported a 50% reduction in estradiol (a form of estrogen) in
    postmenopausal women after lifestyle modification (exercise and high-fiber, low-fat diet) and subsequently noted reductions of 25 and 22% during the follicular and luteal phases, respectively, in premenopausal women after 2 months of high-fiber, low-fat diet intervention carried out at the UCLA Clinical Research Center.

    Breast self examination
    Having a breast augmentation or breast enhancement procedure also offers a perfect opportunity to examine one's own breasts. Performing regular breast self exams can help women become familiar with the individual characteristics of their breasts and notice any changes.
    In recent years, organizations such as the American Cancer Society have changed their positions, no longer recommending that women perform self exams on a monthly basis. This decision was made after several studies revealed that the exams do not increase the chances that women will survive breast cancer. Despite the change in position, the American Cancer Society and other organizations suggest that self exams can be performed to help detect changes in the breasts to call to a physician's attention. Women who perform self exams should consult a healthcare professional to determine how to correctly perform the exam. If women learn to perform breast self exam correctly, they can help detect changes and bring them promptly to a medical professional. Finding a breast cancer tumor when it is smaller improves the chances of saving a breast by avoiding a mastectomy, and may reduce the need for chemotherapy.

    For those patients who do not desire or do not require cosmetic surgery, Dr. Miller utilizes the latest technology and performs the latest techniques available to help improve your appearance. As the body ages, the underlying tissues that keep our skin looking youthful begin to break down, leaving laugh lines, smile lines, crow's feet and facial creases. To help restore a smoother, more youthful appearance, many of Dr. Miller's patients utilize Botox® injections as well as soft-tissue fillers such as Restylane®, Radiesse® and Sculptra®, as well as fat injections.

    Botox® works by reducing the contractions of the muscles that cause those persistent frown lines and creases that have developed over time. It is injected into the skin to treat mild to severe frown lines between the brows, forehead lines, crows feet lines, drooping corners of the mouth, lip lines and neck folds.     Laser therapies increase the thickness and tightness of the skin which allows for regaining or maintaining a youthful appearance of ones face. Dr. Miller offers various technologies each of which encourage cells to make new protein such as collagen.     3-Dimensional Laser:

    Smooth Shapes:

    Nutrition/importance of-under development

    A number of behaviors influence health, such as diet, smoking and physical activity habits. Of these, diet is the most important determinant of health. There is also some evidence that nutrition plays a role in stress.

    Science has provided ample support for the premise that making positive dietary adjustments can influence both present health and future health. A healthful, balanced diet can maintain healthy weight, reduce risk factors for chronic diseases and lower mortality rates. Some studies show that the influence of diet is stronger than other factors such as physical activity, and in some cases dietary improvement may be more effective than prescription medications. For example:
